St. James/Archdiocese of Milwaukee Release of Information Form

I consent to the use by St. James Parish of any photographs, videos and/or audio in which any family members may appear. I understand that these materials are being used for promotion of St. James Parish. Such promotional activities extend to recruitment, fund-raising, evangelization and other communication efforts. I release the staff, volunteers, etc. of St. James Parish from any liability and understand and agree that the use of any family member's picture is not an invasion of privacy. Neither I nor anyone claiming to be speaking on my behalf will later object to St. James Parish's use of these photographs, videos or audios.

I give permission to have my family's contact information released to catechists when needed.

I give permission for St. James to release my family's contact information to the Archdiocese of Milwaukee so I may receive promotional information on Archdiocesan family/child/youth events.
